MOVE Every Heart MKE Program Overview
Health Connections Inc. (HCI), in collaboration with Y-Eat Right, leads MOVE Every HEART Milwaukee through the Milwaukee Heart Health Learning Collaborative (MHHLC). This initiative brings heart health screenings, education, and community resources directly into Milwaukee’s most impacted ZIP codes through a 5-stop church tour.
Target areas: Census Tract 1860 & ZIPs 53205, 53206, 53212, and 53223.

​Core Goals​​

• ​ Connect residents to heart health care through MHHLC member clinics.
• Decrease the rate of uncontrolled hypertension through early intervention and sustainable management.
•  Assess and address social drivers of health (SDOH) that affect heart health outcomes.

• Free Prostate & Breast Cancer Screenings
• Health Education and Wellness Materials
• Hands-Only CPR Demonstrations
• On-Site Medication Reviews
